What is a Community Health Worker ?
Community Health Workers are a vital bloodline of the healthcare system in identifying client and community needs. CHWs are deeply rooted in the communities they serve and have a first-hand look on where the needs lie the most. CHWs are trained advocates of change for the community and clients served to improve access to the quality of care, build cultural competence in services delivered, and ensure health literacy. We believe without the help of our Community Health Workers, our served communities and individual’s health knowledge and self-sufficiency would not be possible.
What We Do
- Serves as a liaison for clients 60+ living in Guilford and Forsyth Counties with complicated medical, mental, social, or behavioral impairments.
- Helps clients access community services and resources.
- Makes home visits and meets clients in health care settings to identify barriers that negatively impacts clients' health and well-being by conducting assessments to document clients' physical, social, behavioral, and spiritual needs.
- Coaches clients to adopt healthy behaviors.
- Facilitates communication with health care providers and social service organizations.
- Provides social support to clients served.
