PTRC AAA Form Certification of Match
This form certifies the required HCCBG 10% matching funds is provided by by the submitting agency.
HCCBG Allocation Request This form gives the Aging Planning Committees a comparison of current funding versus the requested amount for the new fiscal year.
New Agency Requirements as of April 2024 Before a potential new provider can receive funds administered by the Piedmont Triad Regional Council Area Agency on Aging (PTRC AAA), this information must be provided.

Special Eligibility Documentation Form For Nutrition Services This document need to be completed and fully executed, and placed in each special eligible client file.

 

SFY 2024 HCCBG Revision Forms

Revisions are discussed at the local planning committee.  With necessary guidance the funded partner submits the above paper work to the attention of Quality Assurance (unless a designated person has been assigned). This documentation needs to be received at the PTRC office with a pen and ink signature. Electronic submissions are not acceptable, except in extreme cases such as natural disasters or pandemics which necessitate the closure of offices. 

There are two types of revisions.

  1. The first revision is the usual and most common revision and that is a change or movement of allocation from one service to another, with no increase in rate. Usually when revising in the middle of a fiscal year, rates do not change. This is because the rate is set at the beginning of the fiscal year by the funded partner completing the 732a Cost Computation Worksheet which calculates the rate for the entire year.
  2. The second type of revision is the rate change revision. At times there can be instances when a provider needs to adjust their rate in the middle of the fiscal year. When this happens, the AAA Director must approve the rate change, as any change to the rate becomes retroactive to July 1, or the beginning of the fiscal year and will affect all previous payments to the service provider.

 DAAS 21-01_Federal and State Acquisition-Improvement Process

FEDERAL AND STATE ACQUISITION/IMPROVEMENT PROCESS
Purpose: All capital acquisitions and building improvements using federal funding costing $5,000 or more must be pre-approved per ACL guidelines. This has not been routinely followed across all services.
Although this pre-approval is not a legal requirement for purchases using state funding, the Division believes that it is a best practice to put a policy in place that aligns with the federal requirement. Therefore, we are putting a pre-approval policy in place for both federal and state funding purchases exceeding $5,000 when OAA HCCBG funding or state monies are used.
Guidance. Please note that in accordance with NC Administrative Code 01 NCAC 05 and ACL, agencies are to comply with competitive e-quotes or written quotes for purchases $5,000 or more.
The AAA should review proposals for expenditures costing $5,000 or more using the Prior Approval Request and Justification Forms. Once these forms are completed, they should be sent, along with at least three (3) quotes, to Leslee Breen at DAAS. DAAS will then use the Evaluation Checklist to evaluate the proposal for approval.

This 2023 Schedule must be used with all new clients and also with all established clients at the time of their annual reassessments who are receiving Type I Services and are above the federal poverty guideline.

Type I Services include, but at are not limited to, Adult Day Services, Group Respite, Housing and Home Improvement, In-home Aide Services, and Family Caregiver Support – Respite Care and Supplemental Services.
