Take Charge of Your Diabetes

The Diabetes Age Well Services offers a 6-week workshop, originally developed by Stanford University, that has been proven to offer better health outcomes for its participants. We meet both virtually (ask how you can borrow an electronic device!) or in-person once a week for 2.5 hours.

Accrediation_DEAP

Each participant will be connected to a Registered Dietitian to provide group and one-on-one support. Because this is a Medicare Part B benefit, a physician’s referral is required to participate. Every participant will also begin receiving 10 medically-tailored meals a week for an additional 6 weeks at the last session.

Our services are accredited by the American Association for Diabetes Educators and follows the 2017 National Standards for Diabetes Self-Management Education and Support.

Workshops are highly interactive, where mutual support and success build the participants' confidence in their ability to manage their health and maintain active and fulfilling lives.   Diabetes Video
